After reviewing the Berkeley Film Guidelines, please download and complete the appropriate documents below and email them to jchurch@visitberkeley.com. A Film Office representative will contact you once your request has been reviewed.

Please follow these instructions.

1. Download all forms to computer desktop.
2. Open each Adobe PDF document, complete, and save to computer.
3. Email all documents to film@visitberkeley.com & jchurch@visitberkeley.com
